Below are several resources that will assist you and your child through the transition process during their high school years. Please click on the red link for more information.

Transition planning is mandated in the Individual with Education Act (IDEA). The transition planning requirements in IDEA, which include development of an individual transition plan, ensures the planning is initiated in middle school and continued through high school. Transition planning and services focus on a coordinated set of student centered activities designed to facilitate the students movement from school to post school activities, including postsecondary education. It is a collaborative effort and a responsibility shared by  the student, parents, school personnel, and post-secondary personnel.

"Transition Questionnaire"  A set of questions to be completed by both student and parent together intended to assist the school in providing transition services for your son/daughter. Please take the time to print out and complete this Parent/Student Transition Questionnaire. Areas listed will be discussed at the upcoming Individualized Transition Planning (ITP) Meeting/Individualized Educational Planning (IEP) Meeting.

"Summit County Transition Resource Group" A group of professionals that provide transition services to students throughout Summit County. "Disability Disclosure"  Disclosure is a very personal decision, a decision that takes thought and practice. When is to disclose?  Both young people with visible disabilities and those with hidden (not readily apparent to others) disabilities can benefit from accurate information on disclosure. Are you aware of how your disability effects your life? Can you explain what accommodations you need to help you succeed at school, work, or even college?  The following information for young people, and families assists in determining appropriate methods of disclosure.  A workbook is included.
